Family coaches help families find focus and reassurance in their child's age-appropriate development and social-emotional health by providing screenings, tools, tips, and health literacy workshops. Family Coaches also link families with appropriate evaluations and support as needed. Family coaches are committed to helping children and their families build equitable access to information and services to help children and families thrive and succeed. Top candidates will have an ability to authentically connect with children and their families through multiple forms, including but not limited to community outreach, presentations, networking, and electronic and personal communications to build trust and engagement. Primary Responsibilities:
Identifies family's social determinants of health concerns and provides linkage and wraparound services, including linkage to comprehensive care coordination for the whole family
Administers screenings and helps the family understand the results, and provides additional support, information, and referrals
Conducts health literacy workshops
Provide information, referral, and linkage to comprehensive assessments and evaluations based on screening results
Monitors, tracks, and updates child's development
Conducts home, school, and community visits to provide coaching, support, and assistance in navigating evaluation and access to care for Facilities team collaboration with community partners and across programs for seamless wraparound whole family care.
Facilitates activities to strengthen family protective factors and health literacy to support child development and social-emotional wellbeing
Represents ConnectFamilias at Community Events
Celebrate family milestones and assist in removing barriers.
Celebrate child's milestones
Track, document, and report progress towards goals, adjustments to family plan goals, referrals/linkage to care, and outcomes
Participates in required training and team meetings, and case staffing
Success Coach Requirements:
Bachelor's degree in counseling or social work, public health or related field, or Certified C.H.W. with at least two years related experience working with families in communities
Ability to work as a team member and independently
Excellent paper and electronic recordkeeping to meet program reporting deadlines
Exceptional professional interpersonal, verbal, and written communication skills.
Proficient in office software, such as M.S. Office suite, Zoom, DocuSign, and Adobe PDF.
Commitment to participating and implementing professional development training for continuous quality assurance and improvement of service delivery
Must be able to provide services in English and Spanish.
Must have reliable transportation for local travel
Required Clearance:
Florida Background Criminal Screening and National F.B.I. Background Criminal Screening (Level 2)
