Family and Community Care Coordinator (FCCC)
The Family and Community Care Coordinator will work with families at risk of child maltreatment; families with children with behavioral problems, families exposed to domestic violence; families with behavioral/emotional problems, families with children with developmental needs and/or other complex family dynamics that may place their children at risk. The FCCC leads a care coordination team to access the families needs, develop a family plan along with the family and wraparound services to ensure positive results for the children and their caregivers. The FCC will coordinate strategies and linkages to services with formal and informal network of support and community resources.
Qualifications: The candidate must possess excellent written and verbal communication skills. A bachelor's degree in social work, psychology, human services or related field; experience working with children and families; and knowledge of community resources are required. Bilingual English/Spanish is preferred. Full-Time, Exempt. Salary negotiable based on experience.
The Partnership Associate will ensure the smooth running of the ConnectFamilias community partnership through effective management of systems for information flow, as well as tracking and reporting of data.
Qualifications: The candidate must possess excellent written and verbal communication skills. A high school graduate with some college studies in Social Work, Psychology, or other social science/related field is preferred. Office administrative experience is also preferred. Bilingual: English/Spanish preferred. Part-Time, hourly position. Salary negotiable based on experience.
